FEES
Costs will vary based on type of services. Visit the Services Menu to read details about different services and fees listed briefly below:
- Intake with Caregiver(s): $375 (credited toward evaluation cost if scheduled)
- Neuropsychological Evaluation: Between $2500 and $3500
- Clinical Consultation: $200 per hour
INSURANCE
We are out of network providers with insurance companies. All services are self-pay and due at time of service. We will provide documentation for out-of-network benefits to facilitate a claim with your insurance. We encourage you to contact your insurer to ask about out of network benefits. Please use the following procedure codes (CPT) for Intake and Evaluation, if requested by your insurer:
96116 Neurobehavioral status exam
96136 Neuropsychological testing, first 30 minutes
96137 Neuropsychological testing, additional 30 minutes
96132 Neuropsychological evaluation services, first 60 minutes
96133 Neuropsychological evaluation services, additional hour

Accepted payment methods include debit cards and major credit cards (Visa, Mastercard, Discover, American Express), Health Savings Account debit cards (as long as the card is issued by major credit card company), Zelle, and ACH transfer or electronic bank transfer (must clear 3-5 days in advance).
